Supervision: Hours of work:

Health Supervisor

37.5 per week 8:30 -4:30 p.m. Monday to Friday

 

 

PURPOSE:      This position has lead responsibility for developing and managing the Home and Community Care program. Developing programs, annual work plans, budgets, and completing all reporting requirements. Provides direction to and supervises the program staff. Incumbent must have proven project management skills and have advanced knowledge of the various health care issues affecting First Nation.

 

Duties and Responsibilities; (but not limited to}

 

Administration:

Maintain accurate client files with appropriate documents and financial information Participate in regular staff meetings

Provide monthly activity reports

Understand the need to work within the mandate of the Home & Community Care Program Participate in meetings with Health Committee, Band Council and committees as requested. Develop and maintain a positive and professional working relationship with external service        providers ( SWLHIN, MOHLT, Health Canada)

Participates  in  programs  Selection  and hiring of PSW and homemakers.                            

Financial:                                                                                                                                         

Work in conjunction with Health Supervisor to ensure financial accountability for the Homemakers program complete EDRST reports

Process and complete payroll

Prepare bills for payment, maintaining accurate records on file

Ensure accurate and timely completions of all financial monthly reporting requirements to all sources of government funding (i.e. Health Canada, LHIN, Ministry of Health)

Ensure reporting requirements are met in accordance with Federal/Provincial funding agreements.

Ensure budgets are in-line with reporting requirements Sign off on purchases orders on behalf of the program. Develop budgets and annual works plans.

 

Assessments

Provide an objective non-judgmental approach to client’s needs

Coordinate Bi-weekly staff meetings to review duties and care plans as required

Identify on going needs of elders and persons with disabilities and acts as an advocate to meet those needs in conjunction with the HCC Nurse

Assist clients to access goods and services such as home repairs, medical supplies and assist with finding alternate resources if required.

Participate in case conferencing when required.

 

Supervision;

Provide guidance and some supervisions to volunteers working in the program, as requested by the supervisor.

Provides supervision, guidance and leadership to workers under his/her direction. Schedule PSW’s and HCW’s work calendar in consultation with the Home and Community Care Nurse.

Working Conditions:

Work has a high public profile, extensive public interaction and is subject to deadlines

Work requires the ability to take direction, prioritize, work independently and cope with many demands and time constraints

Work may require travel from time to time

Work requires the requirement to work variable hours, when required

The Home & Community Care Program Coordinator will promote courtesy, cooperation and teamwork with HCC staff, including portfolio holders and/or as identified

With External Agencies

Represents and promotes the Health program and seeks to develop sound professional working relationships.

 

Qualifications

 

Education and experience

Post-Secondary diploma/degree in business administration, or related discipline in Office Administration, Business Administration or Office Communications an asset.

Experience in First Nations Health Administration

Knowledge and understanding the community and their needs/concerns.

Reporting to the Social Services Administrator, the Homemaker will provide laundry to clients requiring that service in the home.

Duties:
• To provide laundry service to homemaker clients, which are required to maintain a safe and comfortable environment
• To wash, dry and fold laundry
• To respect the client’s rights, person and property
• To treat all client, related information as confidential
• To document care/service on the daily activity sheets
• To promote client safety and immediately report safety concerns and/or emergency situations by phone to the designated supervisor
• To complete all other tasks as assigned
Qualifications:
• Ability to read, write and follow oral and written directions consistent with job requirements
• Knowledge and experience with cleaning, housekeeping, laundry and other related tasks
• Demonstrated interpersonal skills
• Dependable with good judgement skills
• Current Basic CPR/First Aid certification required
• Must possess reliable means of transportation
• Must provide Criminal Record Check and Vulnerable Sector Check
